Anyone in England can register with a GP surgery. It’s free to register. You do not need proof of address or immigration status, ID or an NHS number.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Not in our catchment area?
The practice generally covers patients who live in or around a six mile radius of Henley. If you live within this area you are welcome to register with us. For more information on the Practice Boundary click here.
You will need to complete the required forms as shown below either using the online form or you can download and email to us at [email protected]. Our reception staff will be happy to help you with any queries.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Would you rather register with a paper form? Download, print, complete and deliver to the practice in person.
If you do not live within our practice area, acceptance onto our list is at the discretion of the Doctor and we will be unable to offer you Home Visits. Please note that your medical records may take several weeks to arrive at the Practice.
Please allow 7 days before your registration will be complete. You will receive an email from us confirming your registration. If you prefer to register in person please call the reception team.

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
